Information, not advice
Covered Mind publishes general information about insurance coverage, costs and clinics. It is not medical advice, legal advice or a guarantee of coverage. Decisions about treatment belong to you and your clinician. Decisions about coverage belong to your health plan.
Accuracy
We work hard to keep information current and we show the source and date for facts. Policies, prices and clinic details change without notice. Confirm anything that matters with the clinic and your plan before relying on it.
Connecting with clinics
When you submit the contact form you ask us to share your information with the clinic or clinics named, and you consent to their contacting you as described in the consent notice. Clinics are independent businesses. We do not control their care, scheduling, pricing or privacy practices.
Clinic accounts
Clinics that claim a listing agree to keep it accurate, to use patient information only to respond to the patient's inquiry, to comply with applicable privacy and telemarketing law, and to the pricing shown at signup. Patient inquiries are billed per the clinic agreement; disputes must be raised within 72 hours with a reason.
Acceptable use
Do not scrape, overload, or attempt to access data you are not authorized to see. Do not submit false information. Do not use the site to harass clinics or patients.
Intellectual property
The site design, text and data compilations are owned by Covered Mind, Inc.. Third-party trademarks belong to their owners and are used descriptively.
Limitation of liability
To the fullest extent allowed by law, Covered Mind, Inc. is not liable for indirect, incidental or consequential damages arising from use of the site, and our total liability is limited to $100. Some jurisdictions do not allow these limits.
Governing law
These terms are governed by the laws of Delaware. We may update these terms; continued use after an update is acceptance.
