The consent text
I agree that Covered Mind may share my information with the clinic or clinics named on the form and that they may call or text me about treatment. Message and data rates may apply. I can opt out at any time.
Version 2026-09-01.v1. We store the version, time and IP address with each submission.
What it means
- We send your name, contact details, zip code, insurance type and treatment interest to the clinic or clinics named on the form, and no one else.
- The clinic may call or text you about treatment at the number you gave, including with automated dialing or texting systems.
- Consent is not a condition of receiving care. You can contact any clinic directly using the phone number on its page instead.
- Message and data rates may apply. Message frequency varies.
Opting out
Reply STOP to any text to stop texts from that sender. Tell a clinic that calls you that you no longer wish to be contacted. Email support@coveredmind.com to have us delete your submission and notify the clinic.
Our emails to you
After you submit, we send up to three emails: what to expect, how prior authorization works, and a check-in a week later. Every email has an unsubscribe link that works immediately.
