Coverage summaries

Each coverage page summarizes one insurer's written medical policy for one treatment and plan type. We link the policy document, record its effective date, and list the criteria in plain language. A licensed clinician reviews each summary before it is marked reviewed, and the reviewer's name and date appear on the page.

Our pipeline re-reads every policy document monthly. When the text changes, the page is marked "under review" and a change post is published. The summary is not marked reviewed again until a clinician re-approves it. A summary that has not been reviewed in 12 months is removed from search engines until it is.

Clinic listings

A clinic is marked verified only when one of these is true:

1. It appears in the Spravato certified center locator (Spravato only).
2. Two independent sources agree on its address and at least one treatment.
3. We confirmed the listing by phone.
4. The clinic claimed the listing and proved control of its website domain or phone number.

Treatments, insurers accepted and prices come from the clinic's own website. Our extraction keeps the exact sentence it relied on and the page URL; a fact without a quote is discarded. "Taking new patients" is shown only when the clinic's site or the clinic itself says so, with the date we checked. Otherwise we say it is not confirmed.

Cost estimates

Insured cost ranges are computed from published Medicare allowed amounts, typical commercial allowed amounts, and standard coinsurance and copay structures. Cash price ranges are the low, median and high of published clinic prices in a metro or nationally. Every cost block states its basis. We update cash ranges monthly and publish a change post when a median moves more than 10 percent.

Paid listings

Clinics can pay for a featured or exclusive listing, and pay per patient who asks to be connected. Paid listings are labeled, limited to one at the top of a list, and never change a clinic's verification status, facts or prices. Unpaid verified clinics always appear.

Corrections

Clinics and patients can report an error at support@coveredmind.com. We correct verified errors within two business days and note the change on the affected page.
